Disaster Medicine Fellowship
Approval of an Disaster Medicine Fellowship program is contingent on a review of the institutional environment, academic productivity of prior graduates and of current members of the team, and the proposed curriculum for future fellows. All approved institutions must have an ACGME-approved residency program in place, and must meet the criteria as set forth in the Request for Applications (see below).

This form is a modified version of the Accreditation Council for Graduate Medical Education (ACGME) Program Information Form (PIF). The ACGME website provides support materials on how to complete a fellowship-specific PIF.
Next Application Deadline: Ongoing with reviews in May and in October each year.
Fellow registration is not required.
Annual Fellowship Evaluations are required of approved programs.
